It's anyone's guess! I don't think it can be explained away. Bottom line is that MI5 should have been keeping tabs on any potential field officer, which is something they apparently neglected to do. Unless Vaughn happened to be owed a favour from someone fairly high-up in the organisation, I don't see how Lucas-really-Lucas could have been killed without anyone realising

I think the more pressing issue is how did no one recognize that he isn't the real Lucas?

Am I to believe the real Lucas North met no one during this whole application process? That no one saw his face? No one checked up on him?

Even if Lucas/John erased all that information and was a good actor, how would he know who saw the real Lucas or not?

The whole concept of Lucas/John being able to pretend his way into MI5 is just ludicrous!
